Obstructive sleep apnea (OSA) is a common disorder characterized by recurrent upper airway collapse during sleep, leading to significant health consequences. Surgical management has evolved considerably over the past decades, shifting from traditional single-level procedures to individualized, multilevel, and minimally invasive approaches guided by advanced airway assessment techniques. Recent innovations, including drug-induced sleep endoscopy, transoral robotic surgery, hypoglossal nerve stimulation, and refined skeletal advancement procedures, have expanded the therapeutic options for patients who are intolerant of or noncompliant with continuous positive airway pressure (CPAP). This presentation reviews the evolution of surgical strategies for OSA, highlights current evidence supporting modern techniques, and discusses patient selection, clinical outcomes, and future directions in precision sleep surgery. Emphasis will be placed on integrating surgical interventions within a multidisciplinary treatment approach to optimize patient care and long-term outcomes.
