EMG Digital Evaluation: Immediate and Sustained Effects of Occlusal Splints

Abstract

This abstract evaluates the immediate and long-term neuromuscular impacts of occlusal appliance therapy on the masticatory system.

 Purpose:

To analyze the immediate and sustained effects of occlusal splints on the electromyographic (EMG) activity of the masseter and anterior temporal muscles during maximal clenching and resting states.
The study utilizes digital EMG evaluation tools to measure changes in muscle equilibrium, symmetry, and overall electrical output over time.

Methods:

Subjects: Patients diagnosed with sleep bruxism or temporomandibular disorders (TMD) exhibiting localized muscle pain and fatigue.
Intervention: Fabrication and delivery of stabilization splints (such as the Michigan splint) via digital CAD/CAM workflows or conventional methods.

Measurement:

Digital surface electromyography (sEMG) recorded muscle activity at baseline (pre-insertion), immediately following appliance delivery, and at multi-month follow-up intervals (sustained effect).

Results:

Immediate Effects: Initial insertion of the splint induces an immediate alteration in jaw biomechanics. It re-equilibrates static neuromuscular relationships, enhances muscular symmetry (Percentage Overlapping Coefficient), and can reduce acute hyperactive electrical activity during clenching.
Sustained Effects: Prolonged use over 3 to 6 months yields a statistically significant decrease in baseline EMG activity during rest and tightens muscle coordination. This chronic adaptation correlates with an incremental reduction in patient pain scales and regularizes muscle voltage curves to mirror healthy baselines.

Conclusion:

Digital EMG evaluation confirms that occlusal splints provide immediate neuromuscular stabilization and sustained therapeutic relief. The therapy effectively down-regulates hyperactive masticatory muscle paths, facilitating a balanced, symmetrical recovery for TMD and bruxism sufferers.
